Provider Manual

Requirement for Submission of CMS Rate Letter(s)

Last Updated on July 16, 2025

Security Health Plan’s reimbursement for inpatient and outpatient services is based on the critical access hospital (CAH), swing bed or rural health clinic's (RHC) most recent rate letter from the Centers for Medicare and Medicaid (CMS). Security Health Plan is not able to access facility rate letters and needs to receive these from providers.

Security Health Plan requires Critical Access Hospitals, Swing Bed and Rural Health Clinics to provide Security Health Plan with their CMS Rate letter no less than every twelve (12) months or within thirty (30) days of the date it receives (whichever is earlier).  Security Health Plan shall apply the new rates to the dates of service no later than sixty (60) days after the date it receives the letter from the Facility or Medicare effective date, whichever is later.  Security Health Plan will not adjust the Critical Access Hospital or Rural Health Clinic’s compensation retroactively unless the CMS Rate letter indicates a decrease in reimbursement.  In this instance, Security Health Plan will reprocess claims back to the effective date of that CMS Rate letter.

The facility should send a copy of its most recent rate letter from CMS by faxing to 715-221-9874 or by email at Dennee.jennifer@securityhealth.org.
