Health Risk Assessment

All newly enrolled Security Health Plan Medicaid members are encouraged to complete a health assessment  within 90 days of enrollment.

The health assessment is intended to identify members who may have high-risk health care needs and provide a baseline health status for care management programs. The process allows Security Health Plan to refer members into three areas of care:

  • Case management
  • Disease management
  • Wellness management

Newly enrolled BadgerCare Plus Medicaid members are encouraged to register for their My Security Health Plan online account to access the online WebMD health assessment. Members can go to for information on how to create their account and complete their health assessment. Those who do not have online access can request a paper copy  by calling Customer Service at 1-800-791-3044 (TTY 711).

Completed health assessments are used to identify high-risk members. These members may be contacted by a health coach to assess their needs and offer associated programs.

Daily Habits
You’ve taken your health assessment. You’ve received your personalized action plan. Great. Now start reaching your goals.  Daily Habits can help you form and maintain long-lasting habits. Just answer a few questions about your health, personal interests and current habits. Daily Habits will provide you with suggested activities to give you a path to better health. Whether your goal is to lose weight, quit tobacco, decrease stress or better manage a chronic condition like diabetes or high blood pressure – Daily Habits has a program for you.

Symptom Checker – Use the interactive symptom checker to see what could be causing your symptoms. Learn about causes, treatments and related conditions.

Health Topics    Get  in-depth assessments and information about specific health conditions like depression, chronic fatigue and diabetes.

Mobile Applications – Access WebMD resources on the go using your smartphone or mobile device with the Wellness at Your Side app. This free app can be downloaded from the Apple App and Google Play stores.
