Wellness and Health Promotion program

The goal of the Wellness and Health Promotion program is to focus on primary prevention by providing targeted interventions that reduce the number of health risks associated with modifiable health behaviors, such as tobacco and alcohol use, obesity, stress, poor diet and physical inactivity. Members are primarily identified for wellness services through the online health assessment. Members can also be referred by RN Care Managers, providers, family and/or self. All members over the age of 18 are eligible for the following services:

  • Online health assessment
  • Digital health coaching
  • Library of electronic health information
  • Telephonic health coaching
  • Telephonic Tobacco Free program
  • Telephonic weight loss program
  • Targeted behavior change mailings
  • E-newsletters
