Chronic Care Management

Security Health Plan offers a Chronic Care Program for members over the age of 18 who have a diagnosis of asthma, diabetes, coronary artery disease, heart failure, chronic lung disease and/or hypertension. The program focuses on promoting member self-management of their condition and coordinating needed health care services.

Members in the program receive targeted educational mailings that address pertinent gaps in care and how to care for their condition. High-risk members may be outreached by a care manager. Care managers provide the following care to members who enroll in the program in order to optimize their self-management skills.

  • Regular contact with a care manager
  • Clinical assessments of health status and comorbidities
  • Support providers’ plan of care
  • Regular updates for provider(s)
  • Make referrals to social workers, health coaches and community resources
  • Perform medication reconciliation
