Health Risk Assessment

All newly enrolled Security Health Plan Medicare Advantage members are encouraged to complete an HRA within 90 days of enrollment.

The HRA process is intended to identify Medicare Advantage members who may have high-risk health care needs and provide baseline health status for care management programs. The process allows Security Health Plan to refer Medicare Advantage members into three areas of care:

  • Case management
  • Disease management
  • Wellness management

The HRA tool is mailed to each newly enrolled Medicare Advantage member along with the “Letter for Confirmation of Enrollment.” The HRA is prefaced by a letter of explanation to the member, requesting their prompt completion of the short health questionnaire, along with instructions for obtaining assistance, if necessary. A convenient stamped, addressed envelope is provided for return of the HRA to Security Health Plan.

Security Health Plan staff follows up with the member if the HRA is not returned, contacting the member directly to assist in the HRA completion if necessary. Medicare Advantage members have the right to refuse to complete the HRA. A copy of the HRA tool is contained in the Appendix.

The returned HRAs are reviewed and those Medicare Advantage members identified as high-risk are referred to the case manager or disease management staff as indicated, to assess the need for case management, disease management or wellness interventions. The members whose HRAs indicate tobacco use and/or no advance directive are sent educational packets with targeted information.

Medicare Advantage members are also sent a modified HRA with their annual birthday card. Returned HRAs are reviewed for triggered responses, with care management follow-up as needed.

Online wellness programs

Health assessment
All Medicare Advantage members are encouraged to complete a health assessment within 90 days of enrollment and one time each year you are a Security Health Plan member. The health assessment asks basic questions about your diet, exercise, sleep, health history and lifestyle. In about 10-minutes you will receive a complete picture of your health and a plan to help reach your best self.  You can also talk with a health coach to get the support and advice you need to move forward with your  health and wellness goals.
