Vision and Hardware Coverage

Vison Exams:

Security Health Plan covers one routine, preventive eye exam and refraction per calendar year. These services are covered with no member cost share when received from either in-network or out-of-network providers (excluding Ally Rx D-SNP which requires use of network providers).

Any additional exams or refractions will be subject to applicable member cost-sharing. To view a member’s eligibility and benefits log in to the SHP Provider Portal. For plan specific documents such as the EOC, see https://www.securityhealth.org/brokers/document-library/medicare-documents.

Cataract Specific Info:

Security Health covers the first external lenses (contacts or glasses) following each cataract surgery; with or without the insertion of standard intraocular lens (IOL). If two separate cataract operations are needed, members cannot reserve the benefit after the first surgery and purchase two eyeglasses after the second surgery. Coverage is for one standard set of frames and lenses or one set of contact lenses with appropriate Standard Written Order (SWO) from the treating provider. The claim must be billed with an appropriate cataract surgery related diagnosis for the eyeglasses or contact lenses to be covered.

If the member chooses deluxe frames or lenses (i.e., scratch resistant, tint, progressive, transitions, etc.), the entire amount will apply to their yearly vision stipend dollar amount because these services do not follow Medicare coverage guidelines.

Vision Stipend:

Medicare Advantage members have a yearly dollar amount, or stipend, to assist with the coverage of prescription eyeglasses not otherwise covered by Medicare. Non-prescription eyeglasses, including reading glasses and contacts, are not covered; members must pay the full cost for these items. Providers must be contracted with Security Health Plan (there is no out of network coverage), have a current prescription on file and be willing to submit claims on behalf of the member.

Security Health Plan will pay the appropriate stipend amount, based on the member's plan, and deny any amount in excess of the stipend as patient responsibility (PR119– benefit maximum exceeded). Amounts paid by Security Health Plan and amounts denied PR119 do not count toward the member’s maximum out of pocket.

Providers should not collect the full balance of the glasses up-front and, later, refund the member the stipend amount paid by Security Health Plan. Providers may calculate the approximate balance that will be due from the member after the stipend is applied and charge the member that balance.

Stipend amounts differ for each plan and may change each year. To view a member’s eligibility and benefits log in to the SHP Provider Portal. For plan specific documents such as the EOC, see https://www.securityhealth.org/brokers/document-library/medicare-documents.
