Provider Manual

Renal Dialysis

Last Updated on November 08, 2022

Inpatient or outpatient renal dialysis treatments for End-Stage Renal Disease (ESRD) are considered urgent/emergent. They are covered when received from either in-network or out-of-network Medicare certified providers for all Medicare Advantage products (including Ally Rx D-SNP). Services are subject to applicable member cost share.

Other covered dialysis services include: coverage of kidney disease education, self-dialysis training, home dialysis equipment and supplies and certain home support services. For Ally Rx D-SNP these services must be performed by an in-network provider.

Applicable member cost-share applies to telehealth services for monthly end-stage renal disease-related visits, in-person office visits and dialysis treatments in an outpatient/inpatient hospital, or home setting.

Certain drugs for home dialysis including heparin, the antidote for heparin when medically necessary, topical anesthetics and erythropoiesis stimulating agents (such as Retacrit, Aranesp®, etc.) will apply to the member’s Medicare Part B prescription drug cost-share.
