HealthCheck

Last Updated on January 24, 2019

HealthCheck is a preventive health-checkup program for members younger than 21 (not covered for BadgerCare Plus Core members).  These exams apply to children covered by any BadgerCare Plus HMO program as well as traditional fee-for-service Medicaid.  The HealthCheck program covers complete health checkups.  These checkups are important for those members younger than 21.

The HealthCheck health program has three purposes:

  • To find and treat health problems for those younger than 21.
  • To let you know about special health services for those younger than 21.
  • To make those younger than 21 eligible for some health care not otherwise covered.

The HealthCheck program covers the medical care for health problems found during the checkup including medical care, eye care and dental care.

Upon completion of a HealthCheck exam, the HealthCheck provider must give each member verification of the exam.  Security Health Plan requires the use of the pink HealthCheck Verification Card.  A completed pink card indicates the name of the provider completing the HealthCheck exam, the member’s name and the exam date.
