Health Assessment

Last Updated on February 19, 2026

All Security Health Plan Medicaid members are encouraged to complete a health assessment within 90 days of enrollment and then at least one time each year.

The health assessment is easy to complete and provides members with a plan to lead a healthy life. It also helps identify members who may need help in the following four  areas of care:

  • Case management
  • Disease management
  • Wellness management
  • Community resources

There are three easy ways to complete the health assessment:

* Members must be 18 years old to take the WebMD Health Assessment. Younger members must have Medicaid annual HealthCheck checkups at a doctor’s office.
