It's easy, download the enrollment form, enroll online or give us a call at (866) 335-0482 (TTY711) Monday - Friday from 8 a.m. to 5 p.m. A Security Health Plan agent will help you find the best plan for your needs.
Appoint a representative
If you would like a medical professional, family member or friends to be able to speak on your behalf regarding your Medicare plan, fill out the Appointment of Representative form, print it out and submit to Security Health Plan with your enrollment or at any time afterwards.

Our Medicare plans are sold by credentialed agents throughout Wisconsin. Find a friendly, knowledgeable agent who lives in your community.
Security Health Plan and its agents are not in any way connected with the Medicare program. Security Health Plan of Wisconsin, Inc., is an HMO-POS and D-SNP plan with a Medicare contract and a contract with the Wisconsin Medicaid program. Enrollment in Security Health Plan depends on contract renewal.
