In-home health risk check for Medicare Advantage members

Having trouble getting in to see your provider? If so, we may be reaching out to you. 

We want to make sure you receive the care you need to stay healthy. That’s why we’re continuing to partner with CenseoHealth to offer in-home health risk checks to Medicare Advantage members.

CenseoHealth calls members who have missed preventive exams or chronic condition appointments. CenseoHealth offers these members the option of an in-home check. A certified medical professional conducts the in-home checks.

The CenseoHealth service is free to our Medicare Advantage members. It does not change your coverage. If you receive a call from a CenseoHealth representative, we hope you’ll consider participating in this free service.

24-Hour Nurse Line

Our 24-hour Nurse Line is available 7 days a week, including holidays. A registered nurse will answer any of your health questions for free. Call 1-800-549-3174 or TTY 1-800-947-3329 if you’re hearing or speech impaired.

Care My Way®

Nurse on Call

Nurse on Call is a service that is available to members in certain areas. A registered nurse will visit your home and assist with education, safety checks, medication explanation and resources.

Sometimes our care managers conclude that a telemonitor could help manage a member’s condition and prevent readmission to a hospital. The telemonitor is a small device that is put in a member’s home, and monitors vital signs daily. The member answers a few easy yes/no answers. A nurse monitors the results and if necessary will contact the member’s doctor to adjust medications.


Security Health Plan wants our members to rehabilitate in the least restrictive, most appropriate setting. That’s why we partner with naviHealth, a post-acute care coordination agent. Local naviHealth professionals work with skilled nursing facilities and hospitals to help Security Health Plan members get better after they receive they've received treatment for an acute health care problem.
