Clinical practice guidelines for chronic conditions (ADHD, asthma, COPD, depression, diabetes, heart failure, hypertension, lipid management, nicotine dependence) 

Family Health Center has adopted national recognized guidelines to ensure our members receiving high-quality care. These guidelines are intended to be a reference tool for health care professionals when determining the care needs of their patients (Family Health Center members). It can help provide an analytical framework for the evaluation and treatment of patients. The guidelines are reviewed annually to assure that they support the quality improvement tool Healthcare Effectiveness Data and Information Set (HEDIS), Uniform Data Systems (UDS) and other quality outcome measures. Family Health Center utilizes outcome measures to identify which of our members have already received the important care they need and which members are in need of care.
