Exam at Community Based Agencies

Security Health Plan has memorandums of understanding with most community-based agencies like county health departments in its service area. This means members can go to these agencies for HealthCheck exams though members are encouraged to establish a relationship with their PCP. The PCP listed in Security Health Plan’s system receives compliance credit for any HealthCheck exams completed at a community-based agency and billed to Security Health Plan. County health departments that do HealthCheck exams are encouraged to send a copy of the HealthCheck exam form to the patient’s primary care provider.
