Our impressive network allows you to choose from thousands of high-quality medical providers including Marshfield Clinic Health System, Ascension - North Region, Aspirus, Essentia Health, Hospital Sisters Health System (HSHS) hospitals in Western Wisconsin, Mayo Clinic Health System, OakLeaf Medical Network (select providers), Prevea Health – Western Wisconsin locations, St. Luke's (Duluth) and UW Health.
Point of Service (POS)
Independence
The POS plan is set up to provide in-network and out-of-network benefits. Employees have access to the same in-network providers as Tradition (HMO), but are eligible to see any provider they choose. Plan members will pay less in out-of-pocket costs if they visit network providers, but can access non-network providers. In-network providers include: Marshfield Clinic Health System, Ascension - North Region, Aspirus, Essentia Health, Hospital Sisters Health System (HSHS) hospitals in Western Wisconsin, Mayo Clinic Health System, OakLeaf Medical Network (select providers), Prevea Health – Western Wisconsin locations, St. Luke's (Duluth) and UW Health.
Exclusive Provider Organization (EPO)

The EPO plan offers employers competitively priced plans exclusively in a 33-county area. The EPO plans provide a variety of benefit options with lower premiums and lower out-of-pocket costs with access to high quality providers including Marshfield Clinic Health System, Ascension - North Region, Essentia Health, Hospital Sisters Health System (HSHS) hospitals in Western Wisconsin, OakLeaf Medical Network (select providers) and Prevea Health – Western Wisconsin locations
Reliance Vantage
Vantage is an innovative new plan available with the Reliance (EPO) network. Some traditional plans leave employees responsible for paying the full amount for all services until the deductible is met. This can lead to large, upfront out-of-pocket costs. But with a Vantage plan, your employees will only be responsible for copays during the first phase of coverage.
