If you don’t know the name of your plan, you can find it on the front of your ID card or at the top of your Schedule of Benefits. Next, fill in information for one or more fields. Then, click Search for a directory of physicians. Enter zip code to narrow your results.

Find medical supplies and services

Use this page to find a supplier for durable medical equipment, orthotics/prosthetics and, for Medicare Advantage plans, hearing aids. Medicare Advantage HMO added benefits

New for 2017, Medicare Advantage HMO plans include three added benefits.  Members can receive a free preventive dental checkup (select button below), up to two hearing aids for a $500 copay per aid and an allowance for eyeglasses for member with routine vision correction needs. See your Evidence of Coverage for details.

To find hearing aid providers, select Find Medical Supplies and Services, then choose your plan and the facility type, Hearing Aid Provider, from the menu on the right.

For eyeglasses, select Find a Doctor, then choose your plan and the specialty, either optometrists or ophthalmologists, from the menu on the right. You can also select Find a Care Facility, then choose your plan and the facility type, Eye Clinic, from the menu on the right.

With our Provider Directory, finding a doctor is easy.

And generally, you must receive care from a network provider at a network practice to have your medical care covered under your policy, so using the Provider Directory also saves you money.

You can:

  • Select a product from the product drop down menu toward the top of the screen. Leave other fields blank if you want a complete list of all providers, or narrow your search results to see a more defined list of providers; or
  • Use the advanced search to find providers by gender, hospital affiliations, accepting new patients, foreign languages spoken or provider level.

Contact providers directly to receive services or schedule appointments. Their addresses and telephone numbers are listed in the Provider Directory above.

If your search for a specific provider does not return any results, call Security Health Plan Customer Service at 1-800-472-2363 to be sure your provider is in Security Health Plan’s network.

Before seeing a non-participating provider, even if your doctor has referred you to that provider, contact Customer Service to find out whether your services will be covered. Care provided by a non-network provider will not be covered unless you receive prior approval from Security Health Plan, except in emergencies and when urgent care is needed outside the service area.

You are strongly encouraged to contact us to verify the status of the providers involved in your care including, for example, the anesthesiologist, radiologist, pathologist, facility, clinic or laboratory, when scheduling appointments or elective procedures to determine whether each provider is a participating or nonparticipating provider.  Such information may assist in your selection of provider(s) and will likely affect the level of  co-payment, deductible and amount of co-insurance applicable to the care you receive.  The information contained in this directory may change during your plan year.  Please contact 1-800-472-2363 to learn more about the participating providers in your network and the implications, including financial, if you decide to receive your care from nonparticipating providers.
