All ECASD employees and spouses insured through Security Health Plan are encouraged to participate in the health insurance wellness program; participation is required to earn a health insurance premium share reduction effective July of 2021. Retirees are not eligible for the premium share reduction, but are encouraged to complete wellness program activities. Program deadlines for 2020
Step 1: Read and consent to the Notice Regarding Wellness Program by Sept. 30, 2020.
Federal rules require you read and consent to the “Notice Regarding Wellness Program” before participating in the district’s health insurance wellness program and earning the health share premium reduction. The notice can be found in your WebMD online Reward Program.
Step 2: Complete your health assessment by Sept. 30, 2020.
A health assessment is fast, interactive, and in about 10 minutes will give you an overall picture of your current health, along with a personalized plan to keep you at your best. Print and share your health assessment results with your provider at your preventive exam.
Step 3: Complete your preventive exam (including biometric screenings) by Sept. 30, 2020.
Schedule a preventive exam with your primary care provider. Bring the Preventive Exam Results Form to your appointment. The form can be found below in "Resources" and in your WebMD online Reward Program. Employees are responsible for faxing completed forms to 715-221-9239 by Sept. 30, 2020. If your provider noted on your 2019 form that you do not need a preventive exam until 2021, the exam and biometric screening will be waived for you in 2020. You will receive credit for meeting all health targets.
Step 4: Meet all biometric Health Targets, or complete alternate standard activities by Dec. 31, 2020.

*Your provider will decide if you need LDL cholesterol or fasting glucose tests based on their clinical practice guidelines. If your provider decides not to complete one or both of these tests, you will receive credit for meeting the health target(s).
Participants are encouraged to work with their PCP to improve their health status. Your PCP can indicate on the Preventive Exam Fax Results form if biometric screening results outside the health target range are acceptable for you (i.e., your blood pressure is controlled through medication), or if they are working with you on a care plan to meet the health target or quit tobacco.

Call 1-844-696-4070 and ask to speak with a health coach. Security Health Plan coaches will call high risk individuals based on their health assessment and biometric screening results to offer health coaching.
Participants can sign up for on-site coaching sessions by logging in to My Security Health Plan and going to their WebMD Reward Program.
Choose from the following classes offered by the district: Bootcamp, Kettlebell, Yoga and Zumba . After each class ask the instructor to sign your ECASD Exercise Attendance form confirming your participation. After you complete six classes fax the form to Security Health Plan at 715-221-9239.
At Keira’s preventive exam, her LDL cholesterol level was measured at 140 mg/dL. This result is above the health target range of less than 130 mg/dL. Keira also informed her provider that she smokes cigarettes. Because she missed both the tobacco and LDL health targets, Keira must complete two alternate standard activities to earn the premium share reduction. To address her high LDL cholesterol value, she decides to complete six fitness classes offered by the ECASD Wellness Committee. To address her tobacco use, she works with a health coach on a plan to quit smoking. After Keira completes both alternate standard activities she will receive the health insurance premium share reduction.
Benjamin’s provider took his blood pressure reading during his preventive exam. His blood pressure is 136/92, which is above the health target of less than 130/80. His provider also measured his waist at 43 inches, which is four inches above the healthy target range. Benjamin told his provider that he does not use tobacco. Benjamin will only need to complete one alternate standard activity to earn the premium share reduction.
When Jake received his preventive exam, his blood sugar value was 110 mg/dL, which is above the health target range of less than 100 mg/dL. To earn a premium share reduction, Jake must complete one alternate standard activity.
