coverage for:
Physician Services – Office visits and consultations,
medically necessary physical exams, maternity care (pre- and post-natal),
surgery, diagnostic services (including laboratory and x-ray),
anesthesia.
Hospital Services – Inpatient care (includes
stay-in semi private room, surgery, laboratory, x-ray and other medically
necessary services), out patient surgery.
Mental Health Services – Inpatient and
outpatient mental-health care and counseling (including substance abuse).
Other Health Services – Preventive medicine
(periodic checkups, well-baby care, examinations), vision and hearing
examinations, dental accidents to natural teeth, oral surgery, hospice,
chiropractic care, therapy (physical, speech, occupational), durable medical
equipment, home respiratory equipment and services, skilled nursing home care,
