Health risk assessment
The Life Focus health risk assessment is a scientific tool designed to evaluate your current well-being. Health risk assessment surveys are available to Security Health Plan members 18 years of age or older*. To get the full benefit of this survey, the most recent information on your weight, blood pressure and cholesterol levels is recommended. The information you provide is confidential and, by law, cannot be shared with an employer or health care provider.
Self-management toolsThe Life Focus library of health information contains interactive tools such as calculators, assessments and quizzes that will help you shape and manage your wellness goals. Use these tools to learn your current health status, evaluate your health risks and plan for future wellness goals. *Not available for Advocare or Medicaid members. |
