ADHD
Security Health Plan recommends members prescribed medication to treat ADHD attend follow-up evaluations during:
Initial phase: One follow-up visit with prescribing provider within 30 days of beginning medication.
Continuation and maintenance phase: Members on medication more than 210 days should receive two follow-up visits within 270 days after initial phase.
Asthma
Cardiovascular
Depression
Diabetes
Hypertension
Nicotine
