Because of the highly confidential nature of some of the information you will record in this planning guide, it should be kept in a safe place. The individual(s) who will need access to this information need to be advised that you have this planning guide and where it is located.

It is recommended that this guide be reviewed periodically to make sure that the information is current.

Advance Directives

Security Health Plan can provide information about advance directives to our members. This includes information for:

• Declaration to Physicians (Wisconsin Living Will)
• Power of Attorney for Health Care
